Deadly Ballast Tank Blast: A Tragic Safety Lesson
While anchored, a crew cleaned and painted a topside ballast tank during routine maintenance. The tank had been open for days, with oxygen checks showing 21 percent. Two hours into spray painting with epoxy mixed with over 30 percent thinners, a massive explosion ripped the tank apart. Five crew members lost their lives, and three went missing.
Key Causes
- High thinner content in the paint created explosive vapor levels during spraying.
- Ventilation was insufficient, relying only on a fan at the manhole and a compressed air line inside.
- A standard cargo light, not explosion-proof, was used inside the confined space.
Critical Safety Takeaways
- Always recognize the explosion risk when spray painting in enclosed spaces.
- Follow strict procedures in the Safety Management System for confined space painting.
- Review Material Safety Data Sheets to understand flash points, explosive limits, and ignition risks of paint, hardener, and thinners.
